From the Templates module, you can add new Form Templates from the Import or Add button in the upper right. 

 

Contact Prestartr Support for assistance with using the Import functionality on the template dashboard.

When creating a new form template you can link it to the following entities:

  • Companies
  • Jobs
  • People
  • Plant
  • Work Order

Templates linked to certain Entities have access to different information.

When linking a template to multiple Entities, it makes the template available for selection in all selected areas.

To add a new Form Template by hand, follow these steps:

  • Click Add on the Form Template Dashboard.
  • Give the template a Name and select which module to Link to.
  • Click Create.

The Form Template Dashboard

Once you have created a new Form Template, you'll be navigated to it's dashboard.

From here click the Edit pencil in the top right.


When editing a Form Template, you have access to two tabs: Info and Fields.

Legacy Content

Some of the fields on the Info tab are only usable for specific clients - these fields will be removed in the future. 

This includes the Prefix, Version, Notify Managers, and Auto Archive sections.

 

There are four sections on the Info tab:

  • Info: This is where you record the name and enable User Notification or Favourite the template.
  • Template Group: This is where you can add the template to any existing Template Groups.
  • Display Columns: This is where you can setup content or information is display when the form template is used.
  • Form PDF Export: This is where you specify the width (in pixels) of labels on this form template when exporting a PDF. 

If updating anything on this tab, ensure you click Update in the upper right to save your changes.

New form templates are empty after creation. 

 

To add fields and functionality to the form template, click the Field tab.

 

There are a number different field types available - details of which are available below.

Question Fields

Form Template Question Fields

Question Fields are some of the most common and useful fields to add to a Form Template.

Before setting up your Question Field

When adding a Question Field to a Form Template, the following information may be important:

  • What is the question being asked? 
    (This is the Name of the Field)
  • Does the question have a default response? 
    (Automatically select Yes - this is the Prefilled With.)
  • How many options does the question have? 
    (Each option is separated by a special character "|" .)
  • Is the question mandatory? 
    (Check the box for Mandatory.)
  • Is this question only needed when something else happens? 
    (Check the box for Hidden.)
    • Does this question need to reveal other hidden fields? 
      (Check Show/Hide Rules.)
  • Is this question only needed when a form is in a certain status? 
    (Set the Form Status.)
  • Is this question only for certain teams or zones? 
    (Set the User Zone.)
  • Does this question raise a defect? 
    (Set the Defect Configuration.)

 

 

 
 
 

Basic Setup

There are five individual question fields available for your Form Templates. Each has a specific and unique use, primarily for legacy content for certain clients.
What this means, however, is that even though we have five options, you can use one to do it all.

Yes - No - N/A

Lets breakdown how to setup a simple Yes - No - NA.

  • Name: The question being asked.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.
  • Prefill with: The preselected option.
  • Prefill from: Pull a response from another field.
  • Options for a Yes - No - N/A question field:
    • 1:Yes|0:No|2:N/A
      • 1:Yes
        This will display as a green button with the text display of Yes.
      • 0:No
        This will display as a red button with the text display of No.
      • 2:N/A
        This will display as an orange button with the text display of N/A.

The option colours are only visible in the Office.

The numbers can be removed without impacting the functionality of the field, each button will be displayed orange, in the Office interface.

  • Onchange: This is an advanced option.

  • Mandatory: Marks the field as mandatory.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.
  • Form Statuses: Determines if the field will display based on the status of the form.
  • User Zones: Determines if the field will display based on selected User Zones.
  • Show/Hide Rules: Sets up a conditional logic to display the field. See Show/Hide Rules for more detail.
  • Defect Configuration: The default settings to create a Defect with this field.

Advanced Setup

When setting up a Question Field, the advanced options include the following:

  • Onchange
  • Show/Hide
  • Defect Configuration

The Onchange field is only used when assisted by Prestartr Support. If you have a form template that we have created that uses this field, contact support for assistance with editing that field.

The Defect Configuration settings is where can determine the default options when using this question to create a Defect.


The Edit Defect Configuration menu has selections for the following fields:

  • Defect Priority:
    Select from Very Low to Very High.
  • Defect Type
    These are set from the Defect Types section of the Catalogue.
  • Defect Photos Field:
    Select a matching Photos field from this template to upload to the created Defect.
  • Defect Comments Field:
    Select a matching Text field from this template to upload to the created Defect.

Open Defects

When creating a Defect from a Form Template field, keep in mind that once an Open Defect is created for a Plant, additional defects cannot be created until the initial defect has been resolved.

Combo Box Fields

Form Template Combo Box Fields

 

Combo Box Fields are some of the most common and useful fields to add to a Form Template.

When adding a Combo Box to a Form Template, the following information may be important:

  • Do they need to select multiple options?
    (Use a Multiselect Combo Box or enable Multi*)
  • What is the selection? 
    (This is the Name of the Field)
  • Is the selection mandatory? 
    (Check the box for Mandatory.)
  • Is this selection only needed when something else happens? 
    (Check the box for Hidden.)
    • Does this selection need to reveal other hidden fields? (Check Show/Hide Rules.)
  • Is this selection only needed when a form is in a certain status? (Set the Form Status.)
  • Is this selection only for certain teams or zones? 
    (Set the User Zone.)
  • Does this selection raise a defect? 
    (Set the Defect Configuration.)

*The Multi option is available on most Combo Box fields that are linked to a specific area, such as Jobs, Plant, and Forms.

 
 
 

Basic Setup

The setup information available here is for Combo and Multiselect Combo Box fields only. 

  • Name: The name of the selection.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.
  • Prefill with: Prefill the selection with a matching option from the Options field.
  • Prefill from: Not applicable for this field.
  • Options: The options for the selection, seperated with a pipe.
    • One|Two|Three|Four
    • Note: Ensure there are no spaces between each pipe.
  • Onchange: This is an advanced option.

  • Mandatory: Marks the field as mandatory.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.
  • Form Statuses: Determines if the field will display based on the status of the form.
  • User Zones: Determines if the field will display based on selected User Zones.
  • Show/Hide Rules: Sets up a conditional logic to display the field. See Show/Hide Rules for more detail.

 
 

Advanced Setup

When setting up a Entity Combo Box, the advanced options include the following:

  • Default Value Dropdown
  • Multi
  • Select Groups
  • Linked Fields (Job Sub Item)
  • For Period Calculation only (Unit)

The Default Value Dropdown is where you can preselect an option from the related entity, such as a Plant, Job, or Work Order.

The Multi checkbox converts the selection from single, to multiple.

The Select Groups option restricts this field to only the selected Group.

Job Sub Item combo boxes will have a Linked Field - this will be a Job combo box, added earlier in the template.

For Period Calculation Only- this option is used when working with Plant Hire.

The Defect Configuration settings is where can determine the default options when using this question to create a Defect.

When setting up a Combo Box field, the following options are legacy fields that are included for specific clients only:

  • User
  • Deal
  • Asset
  • Test

While they can be added to a template, they will not function.

Text Fields

Text Fields are some of the most common and useful fields to add to a Form Template.
The options available include Alpha-Numeric, Numeric Only, and a special Date Selector field.

  • What is the text field? 
    (This is the Name of the Field)
  • Is the text mandatory? 
    (Check the box for Mandatory.)
  • Does this text come from somewhere else?
    (Use Prefill from.)
  • Is this field only needed when something else happens? (Check the box for Hidden.)
  • Is this checkbox only needed when a form is in a certain status? (Set the Form Status.)

Text Field Setup

While there are multiple Text Fields available, they all function roughly the same.

Using the Text or Long Text field is advised.

  • Name: The name of the text field.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.
  • Prefill with: Not applicable for this field.
  • Prefill from: Not applicable for this field.
  • Onchange: This is an advanced option.

  • Mandatory: Marks the field as mandatory.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.
  • Form Statuses: Determines if the field will display based on the status of the form.
  • User Zones: Determines if the field will display based on selected User Zones.

Prefill From:

The text fields are useful for prefilling information on the Job and People on a form.

This includes options such as the Job Name or even a Persons Email.

 
 

Numeric Field Setup

While there are multiple Numeric Fields available, they all function roughly the same.

Using the Numeric or Decimal field is advised.

  • Name: The name of the numeric field.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.
  • Prefill with: Not applicable for this field.
  • Prefill from: Not applicable for this field.
  • Onchange: This is an advanced option.

  • Mandatory: Marks the field as mandatory.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.

Prefill From:

The text fields are useful for prefilling information on the Job and People on a form.

This includes options such as the Job Lunch Break Duration or even a Persons Weekend Rate.

Date Fields are some of the most common and useful fields to add to a Form Template.

When adding a Date Field to a Form Template, the following information may be important:

  • Are you recording the time, date, or both?
  • Do you need it to prefill todays date?
  • If you are recording time, is it the point in time or the duration of time?
  • Is the date field mandatory? 
    (Check the box for Mandatory.)
  • Is this date field only needed when something else happens? 
    (Check the box for Hidden.)
  • Is this date field only needed when a form is in a certain status? (Set the Form Status.)
  • Is this date field only for certain teams or zones? (Set the User Zone.)

This setup covers the Date and Date Timezone Independent fields.

  • Name: The name of the Date field.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.

  • Prefill dates with current day: Defaults the selected date to today. Only for Date fields.
  • Onchange: This is an advanced option.

  • Name: The name of the field.

  • Prefill dates with current day: Defaults the selected date to today. Only for Date Time field.
  • Prefill from: Select time info from Jobs.
  • Onchange: This is an advanced option.

  • Copy Value from Field: Prefill the date from another date field.

Button Fields

Form Template Button Fields

Button Fields are advanced fields added to a Form Template that make other actions occur.

When adding a Button Field to a Form Template, the following information may be important:

  • What should it do? (This is the Name of the Field)
  • Is this button only needed when something else happens? (Check the box for Hidden.)
  • Is this button only needed when a form is in a certain status? (Set the Form Status.)
  • Is this checkbox only for certain teams or zones? (Set the User Zone.)
 
 
 

New Subform and Refresh Subform

The most common Button fields used are the New Subform or Refresh Subform fields.

One will create a linked subform - the other will refresh information from linked subforms.

  • Name: The name of the button.
    • IE - “Create Site Diary”.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.
  • Prefill with: Not applicable for this field.
  • Prefill from: Not applicable for this field.
  • Onchange: This is an advanced option.

  • Action*: Only applicable for New Subform. Select the form template used to create the subform.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.
  • Form Statuses: Determines if the field will display based on the status of the form.
  • User Zones: Determines if the field will display based on selected User Zones.

Open Defects

When creating a Defect from a Form Template field, keep in mind that once an Open Defect is created for a Plant, additional defects cannot be created until the initial defect has been resolved.

 
 
 

Button and QR Codes

Button Fields are advanced elements of a Form Template. Contact Prestartr Support for assistance using a Button Field.

There are two QR Code button fields: Job and External Form.

A QR Code Job field is used for scanning in and out of a Job.
 

QR Code Job

When setting up this button field, it is required to set up two other fields first:

  • Job Combo Box Field
  • Date Time Field

When adding the QR Code Job field after, it will link automatically to the Job and Date Time Fields.

 

 

A QR Code External Form field is used to give access to 3rd parties, such as contractors, to your Prestartr form.


Setting up the External Form field requires you enable Sharing on the form template.

Label Fields

Label Fields are critical layout and organizational fields to add to a Form Template.

When adding one of the Label Fields to a Form Template, the following information may be important:

  • Is it a Header, a Label?
    (Headers can be collapsed, labels display text, and a Score is a legacy feature for specific clients.)
  • Is this only needed when something else happens? 
    (Check the box for Hidden.)
  • Is this checkbox only needed when a form is in a certain status? 
    (Set the Form Status.)
  • Is this checkbox only for certain teams or zones? (Set the User Zone.)

The Header and Label field have the same setup.

  • Name: The name of Header of Label.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.

Formula Fields

The Formula Fields are advanced fields that can be added to a Form Template.

  • Is this result only needed when something else happens? (Check the box for Hidden.)
  • Is this result only needed when a form is in a certain status? (Set the Form Status.)
  • Is this result only for certain teams or zones? 
    (Set the User Zone.)
 
 
 

Setup

The Formula field has four methods of operation.
Normal - Total Hours - Rate - Rate Allowance Amount.

  • Name: The name of the calculated result.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields and for Normal Operation calculations.
  • Position: The order your field display.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.
  • Form Statuses: Determines if the field will display based on the status of the form.
  • User Zones: Determines if the field will display based on selected User Zones.

Normal Operation function like a traditional formula - ((X + Y)Z)). It uses the Label text to point to the right numbers.

Total Hours requires the use of Date Time fields and Duration fields to produce a total sum.

Rate and Rate Allowance both use Date Time and a Sales Service  combo box field to produce their calculation.

When adding a Dependant Field to a Form Template, the following information is important:

Dependant fields are only used with the Work Order Task Combo Box field.

It has two properties available: 

Estimated Duration and Remaining Estimated. Duration.

A Dependant Field is an advanced field in Prestartr - please contact support for assistance with implementing one into a form template.

 
 
 

 

 
 

Signature Fields

    (Set the Form Status.)

Setup Risk Fields

When adding a Risk Field to a Form Template, the following information may be important:

 

  • Have you set up the following:
    • Risk Categories
    • Risk Types
    • Activities
    • Control Measures
  • Should the field prefill from another field?
  • Is the field mandatory? 
    (Check the box for Mandatory.)
  • Is this field only needed when a form is in a certain status? 
    (Set the Form Status.)
  • Is this checkbox only for certain teams or zones? (Set the User Zone.)
 
 
 

Basic Setup

The Risk field is singular in its category.

  • Name: The name of the Risk.
  • Label: The system identifier - this is used when setting up Hide/Show rules for fields.
  • Position: The order your field display.
  • Prefill with: Not applicable for this field.
  • Prefill from: Prefill with responses from another field on the template.
  • Onchange: This is an advanced option.
    See Advanced Setup for more detail.
  • Mandatory: Marks the field as mandatory.
  • Hidden: Hides the field when the form is created. Used with Show/Hide Rules.
  • Prefill: Enabled by default - only disable this at the suggestion of Prestartr Support.
  • Select Risk: The specific risk from the Risk Register.
  • Allow Editing Checkboxes: Enable or disable the ability to edit the initial risk, control measure, residual risk, and Responsible fields on the form.
  • Form Statuses: Determines if the field will display based on the status of the form.
  • User Zones: Determines if the field will display based on selected User Zones.

The Table field is used for Attendance Registers on Site Prestarts.

Before setting up your Attendance Table

When adding a Attendance Field to a Form Template, the following information is required:

  • Job Combo Box
  • Date and Time
  • QR Code (Jobs) Button

 

 

 
 
 
 
 

Edit, Duplicate, and Delete a Field

 

 

You can Edit a field added to the template by clicking the Edit pencil icon.

 

You can Duplicate a field added to the template by clicking the Duplicate icon - the two overlapping squares.

 

You can Delete a field added to the template by clicking the Delete rubbish icon.
